Civil Rights Complaint Form

The American Office of Civil Rights (OCR) is a group that was created by the United States Department of Education to combat discrimination in the public education system. On its web site, an individual can complete a civil rights complaint form if he or she believes that his or her freedoms or rights have been breached. To file the form, an individual must provide a name, an address, an email address, the name and address of the party who was discriminated against and the name and address of the party who is being accused of discrimination. The individual must also provide a description of the believed discrimination.
